Abstract Objective To investigate electrocardiogram (ECG) features of apical hypertrophic cardiomyopathy (AHCM). Methods The clinical data, ECG and echocardiographic features, and the relation between ST- T changes and the thickness of left ventricular apex were retrospectively analyzed in 22 AHCM patients. Results The common characteristic was that marked ischemic changes in ECG were not corresponding to mild clinical symptoms in these cases. Of them, 68% had no related clinical symptoms, 77.3% had mild decline in diastolic function. Al patients had left atrium enlarged and ECG findings of ischemia- like ST segment depression, giant inverted T- wave, tal and broad R wave. The amplitude of inverted T in chest leads, in al leads and R wave in lead V3- V6 were positive relation to the thickness of left ventricular apex (r=0.61、0.64、0.56, al P<0.01). The degree of ST- segment depression was not significantly related to the thickness of the apex (P>0.05). Conclusion The characteristics of ECG in AHCM are giant inverted T wave and tal and broad R wave, which may reflect the degree of apical hypertrophy to a certain extent.
